How to complete a
Beauty consultation sheet

Slide 1 - Slide

Candidate name is YOUR NAME
The clients number is:
The first 3 letters of their SURNAME
The first letter of their FIRST name
The day from their date of birth.
For example, the client code for Blossom Wilson born 25/06/93 would be: WilB25
The date you carried out the treatment
Here you need to record what you are doing. For example, Manicure and nail art
In this area you need to record what units you are being assessed for.
Remember, you do not have to as assessed for everything you are doing.
For example, you may be doing a manicure and nail art but you can just request to be assessed for nail art part of the service.
The time you begin the treatment. You need to record the time for EACH treatment you are carrying out. For example, if you were carrying out a manicure and nail art you must remember to record the time you start your manicure and the time you start your nail art
The time you finish the treatment. You need to record the time for EACH treatment you are carrying out. For example, if you were carrying out a manicure and nail art you must remember to record the time you finish your manicure and the time you finish your nail art.
You also need to make sure you let the assessor know you have finished so they can record the start time too.

Slide 2 - Slide

In this section you need to record what skin type your client has.
Normal: Even skin tone and texture, skin feels soft, firm to touch
Oily: Shiny, Spots (pustules), Blackheads (comedones), large pores, wet/sticky surface.
Dry: Flaky patches, dull, feels tight, small pores, fine lines
Combination: Oily T-zone, usually dry cheeks
In this area you need to record specifically what products you have used. For example, Soothing cleanser, Soothing toner, Ultra soothing moisturiser.
Remember, you might use more than 1 type of cleanser 
Only complete this section if you are carrying out a skin care routine. 
In this section you need to record what tools and equipment you will use to complete your skin care routine.

Slide 3 - Slide

When you are carrying out your skin analysis you need to record what you see on this part of your consultation sheet. Use the key to identify what you have found in each area. 
In the comments section remember to record anything else you notice about the clients skin. For example scars, cuts and abrasions etc...

Slide 4 - Slide

You need to be able to identify what face shape your client has as this will help you to choose the best make-up look for them. If you are not sure what face shape your client has speak to your tutor or look at the face shape handout in the salon.
In this section you need to record what tools and equipment you will use to complete your make up look
In this section you need to record the make and colour of the products you will use to complete your make-up look
Only complete this section if you are doing a make up look

Slide 5 - Slide

Only complete this section if you are undertaking a manicure treatment
In this section you need to record the tools and equipment you are going to use to complete your manicure service
Remember to record what colour varnish you have used to finish your manicure 

Slide 6 - Slide

Only complete this section if you are undertaking nail art
In this sections you need to record the tools, equipment and products you will use to complete the nail art.

Slide 7 - Slide

A treatment plan is a step by step plan how you will carry out your treatment/ treatments.
You need to include any adaptions (changes) to the treatment and explain the reason for the changes
Draw your nail art plan on this nail template
Use this template to show your make-up plan. Remember to shapw your placement of blush, bronzer and contouring

Slide 8 - Slide

In this section you need to get your client to write what aftercare you provided to them as well as a comment on the treatment they received.

Remember t o get them to sign this section too!
In this section you need to record what aftercare you have provided and give the reasons why. 
In this section you need to reflect on the service you have given and record what you think you went well (WWW) and what you could improve on for next time (EBI)
