•  Identify and justify a need for service in a certain community. Explain this need and how you identified it (research).
  
• State link with appropriate subject + explain
  
• Describe your project precisely and briefly
  
•  State other members of your group if applicable
  
• Indicate a realistic time-frame + short-term goals (CALENDAR WITH DATES)
  
•  Enter supervisor details (subject teacher)
  
•  Select no more than 3 to 4 learning outcomes linked to your personal learning goals that you hope to attain through the project
  
•  Description: how you will achieve your aim and each of the selected learning outcomes
