When was the text written? How do you know this?

Now great glorious God through grace of Himself
And the precious prayer of his excellent Mother
Shield us from shameful deeds and sinful works
And give us grace to guide and govern us here
In this wretched world through virtuous living,
That we may go to his court, the kingdom of heaven
And I shall tell you a tale that true is and noble
Of the real royal men of the Round Table
That chief were of chivalry and chieftains noble
Both wary in their works and wise men of arms,
Skilled in their doings and dreaded always shame,
Kind men and courteous and couth of court manners,
How they won in war worshipped by many,
Slew Lucius the Wicked that lord was of Rome,
And conquered that kingdom through crafts of arms;
Listen now here and hear this story!
When that the king Arthur by conquest had won
Castles and kingdoms and countries many,
And he had recovered the crown of that country rich
Of all that Uther in earth owned in his time.
