A form usually asks for your name, date of birth, contact details, and family details.

Let's look at Sara's form. Note details such as her full name, address, date of birth, nationality and occupation.
Full Name and Initials
On a form, a full name can include first name, middle name, and surname. Initials are the first letters of your names.
Address and Postcode
An address tells where you live. A postcode is a special code for your area.
Date of Birth and Age
Date of birth is written as day/month/year. Age tells how many years old you are.
Nationality and Country of Birth
Nationality is your country of citizenship. Country of birth is where you were born.
