What does 'passive' mean?

What makes a sentence passive?
een vorm van 'to be' + ww-ed (3e rijtje) 
It is not important who is doing something

Active: The teacher teaches the student.
Passive: The student is taught (by the teacher).

How to recognize a passive sentence...
- is er een vorm van 'to be' (be, am, are, is, was, were, been)
- staat er een voltooid deelwoord (ww-ed / 3e rijtje)?
- komt daarna 'by' gevolgd door een zelfst nw of persoonllijk vnw? Zo niet, kan er 'by zombies' achter?
Is het antwoord op bovenstaande vragen ja?

ONDERWERP = WIE /WAT het doet staat vooraan de zin.
